USP <800> applies to anyone who handles hazardous drugs as identified on the NIOSH list. It covers facility and engineering controls (such as containment ventilation), personal protective equipment, a written hazard-communication and exposure-control program, deactivation and decontamination, spill management, and medical surveillance. A pharmacy’s <800> compliance is typically evidenced by an assessment of risk, written SOPs, training records, and documented containment.
